Where Questionnaire Answers Live

Every answer to every question gets saved against the individual — never against the questionnaire template itself. This means you can edit, delete, or replace a questionnaire later without losing any historical answers.

Viewing Answers

Open the client's profile and scroll to the Questionnaire Answers card.

You'll see answers grouped by questionnaire name, listed as a simple table:

If the profile has no answers yet, the card shows No Questionnaire Answers with a prompt to start one.

How They're Stored

Answers are stored as one record per question, per individual — not as a single submission blob. This has a few practical effects:

  • Re-running a questionnaire updates existing answers for the same individual instead of creating a duplicate set.

  • Editing the question text in the builder doesn't break old answers — they're linked by question ID.

  • Deleting a question from the questionnaire removes future answers but historical answers can still be queried by question ID.

Same Place, Both Sources

Whether the client filled out the answers in a client portal or you ran the questionnaire from the profile yourself (see Running a Questionnaire from a Profile), the answers land in the same panel. There's no separate "portal answers" view — it's all one history.
